Paediatric Community Nurses


Paediatric Community Nurses (PCN) are a team of dedicated children's nurses and health care support workers who provide nursing care for children and their families, living in the community. The children usually need nursing care but do not need to be in hospital and can have a normal home life.

The PCN's can visit you and your child at home, school or any setting convenient to you, close to home.

The team provide a service to children, from birth up to the age of 16. Although for children who remain under a paediatric consultant, this age limit can be extended to 19 years of age.

The PCN's aim is to support children and their families in their home and other community settings. They can offer a wide range of specialist skills and care for children with chronic, acute, palliative or complex care needs.

They should ensure that your child is as healthy as they can be and can often alleviate concerns. They will have knowledge and experience of your child's condition and be aware of what is 'normal' for your child.

PCN's usually work 7 days a week; they will inform you of when they do work on your initial visit.

PCN's only get involved with you from a referral and this can come via your GP, specialist nurse, Children's A&E or Wards.
